Studio Policies

Please read these policies carefully before using FORM BY TWO services.

CLASS CANCELLATIONS

Class and waitlist cancellations must occur at least 8 hours in advance of the start of class. If you join the waitlist, please pay close attention to communications/notifications up to 2 hours in advance of class time to see if you’ve been placed into class. If you are admitted to the class, We assume you will be attending unless you notify us that you are unable to attend (at least 8 hours in advance). If you late cancel or no-show to your class, you will forfeit the credit used for the class booking. Classes cannot be rescheduled for a different class time within the 8-hour window.

Private sessions must be canceled 24 hours before the session starts. Cancellations within 24 hours will be considered a late cancellation and you will forfeit the session credit. 

CLASS ATTENDANCE & PUNCTUALITY

New Clients: Please arrive 10 minutes before class begins to ensure you receive an introduction to the Megaformer. If you are not present five minutes before class begins, we are unable to allow entry to that class, and you will be rebooked. This is to ensure your safety and that you understand the machine before the class begins. 

All Other Clients: If you are not present at the start time of your reserved class, your spot will be given away to a client on the waitlist. If no one is on the waitlist, you can still attend, however, we have a strict 5 minute policy. After that time, you will be unable to join the class.  After this 5 minute cutoff you will be late canceled from the class. No exceptions. Thank you for your cooperation in advance.

INSTRUCTOR SUBSTITUTIONS 

Instructors are subject to change without notice, and class schedules may be subject to change without notice. When possible, an email or text message will be sent in the event that there is a change in the schedule. 

PERSONAL BELONGINGS

You agree that the FORM BY TWO is in no way responsible for the safekeeping of your personal belongings while you are present in the studio. FORM BY TWO recommends you leave all valuables at home. You assume all risk of loss for any of your personal belongings.

WAITLIST

You are encouraged to join waitlists for classes that are full. The waitlist cut-off time is 2 hours prior to the start of class. Up until the waitlist cut-off time, waitlist participants will be automatically added to the class roster if a space becomes available, in the order they were added to the waitlist.

After the 2 hour waitlist cut-off time, if a space becomes available, all waitlist participants will receive a text message notifying them that a space is available. The first person to respond will receive the spot. 

TACTILE CORRECTIONS

Safety is crucial, and therefore our trained instructors will provide corrective touches during classes to ensure that clients are safe and maintain correct form. Please inform your instructor prior to the class / session if you are uncomfortable with tactile cues / corrections. 

PRIVATE TRAINING 

Use of private trainers or instructors not approved and employed by FORM BY TWO is prohibited at the facility.  You may not train or instruct other members or guests, conduct business activity or solicit any business at the FORM BY TWO facilities.

MINOR’S USE

Minors under the age of 15 are not permitted to take classes in the studio.  Minors ages 15 to 17 are not permitted in the studio unless they are actively involved and enrolled in a class or sessions and have had a parent / guardian sign the Minor Participant Waiver in person at the FORM BY TWO studio facilities.   The parent or guardian does not need to attend the class but must remain at the studio facilities during the class. GUEST USE

Your guests are permitted to use the FORM BY TWO studio facilities, but must adhere to all rules, regulations, fees, and schedules.  FORM BY TWO reserves the right to limit the number of times any one guest can use the FORM BY TWO studio and reserves the right to exclude any of your guests whose use of the studio, in the sole opinion of FORM BY TWO would be detrimental to FORM BY TWO or any of its members or employees.  All guests must complete a Guest Registration Form and sign a Liability Waiver at the front desk with valid identification.

STUDIO RULES AND REGULATIONS

All classes must be prepaid through our online booking system, Momence, available through our website or the mobile app via Google Play or Apple App Store. All reservations must be confirmed.

Membership bookings are limited to 1 reservation per day. Additional reservations must be made using supplemental class credits. 

You must be physically present and checked in at the start of class or your spot may be released to a waitlisted client. No exceptions.

Reserved classes must be changed or canceled at least 8 hours prior to the scheduled time or they will be forfeited.

Membership packages are subject to a penalty fee and/or loss of class credit for a late cancellation or no show.

If recovering from injury or illness, please notify the instructor prior to class start time.

Grip socks are MANDATORY in all classes. No exceptions. Socks are available for purchase at the studio. 

Cell phones should not be used during class. 

Unattended pets and children (under 18) are not permitted in the studio.

Clients are responsible for wiping down their machine with the provided cloths after class.

Subject to any regulations, all credit card charges are subject to a three percent (3%) processing fee. To update your card on file to an ACH payment method, login to “My Account” on the Momence website (https://momence.com/sign-in) or app. Processing fees will not be refunded.

We accept Visa, Mastercard or American Express.

The Company is not responsible for any bank overdraft fees that may occur.

POLICIES FOR MINORS

MINOR’S USE

Minors under the age of 15 are not permitted to take classes in the studio.  Minors ages 15 to 17 are not permitted in the studio unless they are actively involved and enrolled in a class or sessions and have had a parent or guardian sign the Minor Participant Waiver in person at the FORM BY TWO studio facilities.   The parent or guardian does not need to attend the class but must remain at the studio facilities during the class. FORM BY TWO and the instructor has the right to ask the minor to leave the class and/or revoke the minor’s membership if inappropriate behavior is exhibited.

SETUP & MANAGEMENT OF MINOR’S ACCOUNTS

Minor Member accounts must be set up by a parent/guardian in either of the following: (a.) The parent/guardian is an existing member and the minor child is added as a secondary account of the existing member, or (b.) The child’s account is set up as an individual account managed by the parent/guardian.  The parent /guardian and minor child are both required to sign the “Minor Liability Waiver”. The parent /guardian is required to book all classes on behalf of the minor child in either situation.

MEMBERSHIP POLICIES

MEMBERSHIP FREEZES

Freezes are only available for 12 month membership agreements. You can freeze up to 3 months during your 12 month agreement for a $20 per month freeze fee.

Members who provide a doctor’s note outlining their inability to practice, may freeze their membership for up to 6 months with no charge. 1 month prior to the freeze ending, we will re-evaluate the member’s condition to determine future membership.

Freeze requests must be sent in writing via email to info@formbytwo.com.

MEMBERSHIP UPGRADES / DOWNGRADES

If you wish to upgrade or downgrade your membership, your commitment term will restart effective as of the transfer date. Your new membership monthly dues will adjust to the currently available rates. Requests to upgrade / downgrade memberships must be sent in writing via email to info@formbytwo.com.

Exception: In the first 3 months of a Founding Member’s agreement, the member may choose to upgrade their membership and monthly dues will adjust to the founding membership rates available at the time of the original purchase. Downgrades will adjust to the currently available rates.

MEMBERSHIP CANCELLATION

If you would like to cancel your membership, you must inform FORM BY TWO in writing by sending an email to info@formbytwo.com or by hand-delivering a written letter to the FORM BY TWO studio within 5 days before the end of the agreement term. The written communication must include your name, reason for cancellation and date. You must have an active, unfrozen membership to request a cancellation.

You will be responsible for 50% of the remaining billing periods of your agreement. If you cancel within 5 days before the next billing cycle starts, you will be charged an additional $20 late membership cancellation fee.

If you cancel after the next billing period starts, you will be charged for that billing period and still have access to the studio during that time.

Note: Cancellation during a 3-month membership agreement does not entitle you to any refund of any kind for any amount. You will continue to retain access to your membership for the remainder of your term.

CANCELLATION RIGHTS: You may cancel for the following reasons with no penalty if you send a written notice to info@formbytwo.com or deliver a signed & dated notice to FORM BY TWO’s studio at 301 N Harrison St. #263, Princeton, NJ 08540 upon the following:

a.) In the event of death or permanent disability, if the permanent disability is fully described and confirmed to FORM BY TWO by a physician.

b.) Upon the change of your permanent address to a location more than 25 miles from the FORM BY TWO studio facility. FORM BY TWO may require proof of your new permanent residence.

FORM BY TWO may retain the portion of the total agreement amount representing the services used plus reimbursement for expenses incurred in any amount not to exceed 10% of the total agreement amount.

(c) FORM BY TWO closes for a period longer than 30 days: If FORM BY TWO closes for a period longer than 30 days through no fault of your own, you are entitled to either extend the agreement for a period equal to that during which the facility is closed, or to receive a prorated refund of the amount paid by you.
